What is the procedure for installing the side mirror on the door?? Does the inside door panel have to come off in order to install the mirror from UNDER the mounting location on the door??

if you have the remote mirror, the door panel defiantly has to come off.there is a stud on the mirror, and you have to put the nut on, on the inside of the door. never had a manual mirror, but it probably needs to come off as well.

Yes. the door panel has to come off.
It helps to have a panel removing spoon when removing the panels to keep from damaging the sometimes brittle panel backer board. http://www.harborfreight.com/5-piece-auto-trim-and-molding-tool-set-67021.html
Besides the phillips head screw that can be removed from the top; the mirror has a 1/4-20 stud with a 7/16" head nut on it.
I had a car once that the phillips screw hole was stripped from someone over tightening it at some point.
I used a small clip from a old grille underneath the door skin from the inside to hold it in and it worked well.
