Need help decoding a fender tag.

Any help I can get Decoding the fender tag would be appreciated.I know it's not the usual 68-74 Road Runner ..but I am hoping I can get some idea what this thing should have had.. Thanks in advance..

Thank you..That got me a lot closer. Are all years the same code Numbers?
